Technicians Gary Sitterson and Steve Hadley prepare for an operation, one of several taking place around the world, while joking with fellow technician Wendy Lin.

College students Dana Polk, Holden McCrea, Marty Mikalski, Jules Louden, and her boyfriend Curt Vaughan go to a remote cabin in the woods for a vacation. The technicians control the environment the group are in from afar and give them mood-altering drugs to make them more likely to venture into the cellar and do something that will lead to "punishment". The drugs gradually reduce the group's intelligence and awareness, and also increase their libido. After entering the cellar, the group discovers a large assortment of items, including a diary by Patience Buckner, a girl abused by her sadistic family. Reciting an incantation from the diary, Dana inadvertently summons the zombified Buckner family.

Curt and Jules, influenced by more mood-altering drugs, go outside to have sex. The Buckners attack the lovers and kill Jules, but Curt flees to the cabin and informs the group of Jules' death. Marty, who frequently smokes marijuana, becomes paranoid, and believes they are being manipulated. Discovering a hidden camera, Marty thinks that he is on a reality television show, but is attacked and dragged away by one of the Buckners. Holden, Dana, and Curt attempt to flee in their RV, but the technicians are able to trigger a tunnel collapse in time to block their path. Curt attempts to jump a ravine to flee only to crash into an invisible force-field and fall to his death. Realizing that something is unusual about the environment, Dana becomes convinced that Marty's worries about them being manipulated were correct. While driving back to the cabin Holden is killed by a Buckner who has been hiding in the RV. The RV falls into the lake and Dana swims away, only to be attacked by a Buckner on the dock.

The technicians celebrate the completion of the scenario, but a phone call informs them that Marty is still alive. Arriving at the dock, Marty saves Dana. He reveals that he dismembered his assailant after being dragged away, stumbled upon a hidden control box, and was able to access an underground elevator. The technicians realize that Marty is immune to their drugs because of his marijuana habit. Marty and Dana take the elevator down to the lower levels of the facility, passing a variety of imprisoned monsters. One of the monsters carries an item that Dana remembers seeing in the cabin's cellar, and she realizes that the items in the cellar determine which monster will be released. A security team then corners her and Marty, but she uses a control station to release the monsters, which massacre the facility staff. Hadley, Lin, and Sitterson are killed in the carnage. Dana flees to the lower levels of the facility with Marty.

Discovering a temple adorned with large stone tablets, Dana and Marty meet the Director. They are informed that the scenario is to appease the "Ancient Ones" — beings who live beneath the facility and are kept in perpetual slumber through an annual, pars pro toto ritual sacrifice of five young people who embody certain archetypes: the Whore (Jules), the Athlete (Curt), the Scholar (Holden), the Fool (Marty), and the Virgin (Dana). The order in which they die does not matter, as long as the whore is first and the virgin is last, and her death is optional, as long as she suffers. Should the Ancient Ones awake, they will destroy the world. The Director reveals that rituals around the world have been taking place for the same purpose, but each of them failed. The Director urges Dana to kill Marty to complete the ritual. Dana draws a gun on Marty, but is attacked by a werewolf while the Director fights with Marty. Patience Buckner arrives and kills the Director before Marty pushes them both into the Ancient Ones' pit.

Marty forgives Dana for threatening to shoot him. The pair accepts that it might be better for another species to take humanity's place if this is the price of its continued existence. Marty and Dana light a joint and hold hands as the gigantic hand of an Ancient One rises up, destroying the facility and the cabin.

Based on the "real" life events of Maria Rossi and her daughter Isabella Rossi. On October 30, 1989, Maria Rossi (Suzan Crowley) committed a triple murder during an exorcism performed on her. The Catholic Church became involved, and she has since been in a Catholic psychiatric hospital in Rome. The film shows a news story and police investigation showing the three members of the clergy whom she murdered. Her daughter, Isabella, learned of the murders from her father, who died three days after telling her.

Twenty years later, Isabella (Fernanda Andrade) is in the process of filming a documentary about exorcisms and, to find out more about her mother, she visits a school in Rome. She meets two priests, Ben (Simon Quarterman) and David (Evan Helmuth). Ben and David take Isabella with them on an exorcism performed on Rosalita (Bonnie Morgan). They bring along medical equipment to determine if it's possession or mental illness. Rosalita attacks the crew after spouting out obscene remarks in different languages and accents. She calls Isabella by her name, despite not knowing her. Eventually, they get her under control.

When Isabella goes to visit her mother, Maria, in the asylum, she finds that her mother speaks in different accents and has paintings all over the walls. She has inverted crosses carved into her arms and her bottom lip. Maria tells Isabella that killing a child is against God's will, then lets out an ear-shattering scream. Isabella tells David and Ben that she had an abortion years ago and her mother had no way of knowing that—another sign that showed possible possession. As the crew prepares to perform an exorcism/analysis on Maria, David worries about losing his job, since the Church does not authorize exorcisms without undeniable proof that the patient is indeed possessed. During the procedure, Maria mentions knowing what Ben did in the past, as well as Isabella's child. She breaks free of her restraints and sends Ben flying into the door, also knocking David to the ground. The doctors rush in shortly after.

After analyzing the data from the video and audio files, they present the evidence to the Church. David shows many signs of stress, as Ben plays the audio files over and over, listening to the part where Maria says "I know what you did". Ben then finds that there are four different demons speaking in unison in a different clip. David is to perform a baptism at his church, in which Michael tags along to record. The service starts without incident until he holds the baby to start the Immersion Baptism. He then mutters some lines from the Bible and starts forcefully submerging the baby in the holy water, staring blankly at the camera. The crowd of people rush up to save the baby as he passes out.

Soon after, Ben finds David at home with blood all over his forearms, eyes rolled back into his head, much the way Maria was during the exorcism. The police arrive, and though it doesn't show on film, David acquires an officer's handgun and holds it in his mouth. Ben tells him to fight it, but he begins to weep, reciting The Lord's Prayer, stopping; forgetting the last few words. He laughs and then shoots himself. Just then, Isabella begins having a seizure.

Ben hysterically comes to the realization that Isabella is possessed. As they wait in the hospital, nurses rush into the emergency room and find a nurse on the floor, blood spewing from her neck as other nurses fight to restrain Isabella. Ben and Michael drag Isabella into the hallway and restrain her. They leave with Isabella in a car, heading to get help for a potential exorcism. While Michael drives, Isabella speaks of also knowing the horrible act Ben committed, scaring Ben. She then tries to strangle Michael and Ben is able to pull her off before Michael loses control of the car, but not before she breathes into Michael's mouth. He instantly shows signs of possession, and acceleration can be heard as Michael speeds into oncoming traffic, headlong into another car. The camera goes black, and cuts in with short sequences of chaos, most likely of Michael, Isabella and Ben flying through their car windshield, leaving the outcome unknown. After the screen finally cuts to black and the credits start rolling, a title card is shown informing that the case of the Rossi family is still unresolved, followed by another title card directing viewers to a website (www.therossifiles.com) "for more information on the ongoing investigation."

The film opens with an unknown man drawing and organizing diagrams to the song Silent Night. He picks up his parka coat and leaves.
While attending a Christmas party, David (Brian Geraghty) offers to drive his co-worker, Emily (Alice Eve), home. Reluctantly, he allows his friend Corey (Josh Peck) to accompany them. On the car ride, Corey forces David to pull up at a local ATM, wanting to get money for pizza. Corey and David enter the ATM to withdraw money, and shortly after, Emily joins them. When the three are about to leave, they turn to see a hooded figure lurking outside. While Corey suggests they return to the car, Emily and David are reluctant, believing the figure to be a robber. Although they are scared, they realize, though later proven wrong by David, that the figure cannot get in without an ATM card. The hooded figure murders a local man walking his dog, prompting the three to call the police, only to realize they had left their cellphones in the car. While the three are panicking inside, the hooded figure cuts off the heater in the ATM booth, effectively making it colder. David decides to reason with the hooded figure, and attempts to give him 500 dollars, valuable earrings, and a watch in return for their safety. While giving the money to the hooded figure, he runs back to the car, attempting to grab Emily's phone to alert the police. He is chased to the car, gets the phone and realizes the wires have been cut preventing the car from being driven. The hooded figure then attacks him by smashing out the passenger's side window. He escapes the car and is chased back to the ATM booth, unharmed, but drops the cellphone along the way. Emily uses her lipstick to write "HELP" on the ATM booth's window. The hooded man takes the phone, the valuables and the wallet from the dead man.
As the three begin to freeze, a security guard sees the sign and goes to investigate, but is beaten to death by the hooded figure (with a crowbar taken from David's trunk) in the process, leaving them hopeless. Another man with a similar coat to the killer enters the ATM, but Corey and David manage to kill him by strangling him. They later realize that the man was innocent and was only going to work, because he was the janitor. Corey takes his coat and grows angry and tired and attempts to leave the ATM booth, but is caught and stabbed by the hooded figure. After several more hours freezing in the ATM booth, David and Emily realize Corey is still alive. They both run outside, grab him, and manage to return to the ATM booth before the hooded figure gets to them. While David and Emily attempt to stifle Corey's bleeding wound, the hooded figure begins to fill the ATM booth with cold water, attempting to freeze them to death. Corey eventually dies of blood loss and hypothermia. David and Emily concoct a plan to trigger the fire alarm to send for help, using Corey's lighter, a trash can, and paper. Emily climbs on David's shoulders and manages to get the fire close enough to the fire alarm and is successful, but David slips and Emily falls, smashing her head, and dies. The man had driven David's car against the door before filling the ATM with water and now smashes the security guard's car into it. Angered, David attempts to throw a Molotov cocktail at the hooded killer, only to realize that it is not the killer, but the body of the killed security guard. The police arrive and David is arrested.
While David is being driven away, he sees several figures in similar parkas as the killer, still unsure of who the masked madman could be. The police find a video tape recording the events within the ATM, but it is made obvious that the killer had planned his actions so as not to appear in the footage, effectively framing David for the killer's crime. He also planted other evidence against David. In the final scene of the film, the hooded figure, still unknown, returns to his headquarters, where he begins to map out similar attacks on a mini-mart.
